Instructor and Department reports for Spring 2026 semester are scheduled to be released on May 13, 2026.  

Results Reports

Please sign in to Blue https://sfi.wcupa.edu/ to view results reports for Spring 2024 forward. For reports prior to Spring 2024, instructors should request reports from their department. If necessary, please contact Institutional Research at sfi@wcupa.edu to obtain copies of reports prior to Spring 2024.

Reports are issued after the surveys have closed and final grades have posted. Instructors and Department Chairs will receive a system-generated email from SFI@wcupa.edu notifying them that reports are ready to view and download. It is recommended to download and save reports for your personal/department records in the same way that you were accustomed to saving your paper results reports.

Beginning in Fall 2024, reports were redesigned as frequency only reports to be in compliance with APSCUF side letter dated May 29, 2024. As required by APSCUF, results reports will no longer contain means or standard deviations and will only contain a frequency of responses.

An introduction to each report is provided below. Questions regarding survey development, testing, administration, and score reporting should be directed to Lisa Yannick in the Office of Institutional Research.

SFI Student Feedback on Instructor

The Student Feedback on Instructor instrument is comprised of 25 questions. Students are asked to choose from the following response options:

  • Item 1: Outstanding (6) - Poor (1)
  • Items 2-18: Strongly Agree (6) to Strongly Disagree (1)
  • Items 19-22 are open-ended questions inviting student comments.  Student responses to these open-ended questions are returned directly to the instructor after grades have been submitted for the semester.
  • Items 23-25 are demographic and are asked for statistical purposes only. Results are not reported to the instructor or academic department and will only be used in the Office of Institutional Research in aggregate to test the feedback instrument.

Results reports are issued for surveys that receive a minimum of 5 student responses and greater than 50 percent response rate. The results report includes the response count and frequency.

The instructor receives the quantitative results and any student comments collected.  The department chair receives only the quantitative results.

DE Student Feedback on Instructor for Distance Education

The Student Feedback on Instructor for Distance Education instrument is comprised of  23 questions. Students are asked to choose from the following response options:

  • In Item 1 students are asked to rate the quality of teaching provided by the instructor from “6” Outstanding to “1” Poor.  
  • In Items 2-10 and 12-14 students are asked to indicate their level of agreement to statements regarding the quality of instruction from 6 (Strongly Agree) to 1 (Strongly Disagree). These first 16 items correspond to items on the SFI instrument.
  • Items 11 and 14-16 are indicators of the student’s experience with distance education in general and with the use of technology in this specific course section.
  • Items 17-20 are open-ended questions inviting student comments.
  • The final 3 items on the survey are demographics which are collected for statistical purposes only.

STSFF  Student-Teaching Supervisor Feedback Form

The Student-Teaching Supervisor Feedback Form is comprised of 21 questions. Students are asked to choose from the following response options:

  • Item 1:  Students are asked to indicate the Length of Supervision (first half, second half, entire semester).
  • Item 2:  Asks the student to select the Student Teaching Program from 11 choices: English, Foreign Languages, Health & Physical Ed, Sciences, Mathematics, Music Ed, Social Studies, Middle Grades, Early Grades, Special Ed, Comm Dis/Speech Pathology.
  • Items 3 - 16: Students are asked to indicate their level of agreement to statements regarding the quality of supervision from response choices: Excellent, Good, Satisfactory, Fair, Poor, Not Applicable.
  • Items 17 - 20: Require Yes or No answers regarding aspects of supervision.
  • Item 21 is an open-ended question inviting student comments.

Results reports are issued for surveys that receive a minimum of 5 student responses. The results report includes the frequency for the rating scale questions and percent for Yes/No questions.

The instructor receives the quantitative results and any student comments collected.  The department chair receives only the quantitative results.

NCL Student Feedback on Instructor for Nursing Clinicals

The Student Feedback on Instructor for Nursing Clinicals instrument consists of 14 questions. Students are asked to choose from the following response options:

  • Items 1-10:  Students are asked to indicate their level of agreement to statements regarding the quality of instruction from Very Effective (5) to Ineffective (1).
  • Items 11-14 are open-ended questions inviting student comments.

Results reports are issued for surveys that receive a minimum of 5 student responses. The results report includes the response count and frequency.

The instructor receives the quantitative results and any student comments collected.  The department chair receives only the quantitative results.

Student-Athlete Feedback on Faculty Athletic Trainer

Before being presented with any survey questions, the student athlete is asked to confirm that they have received athletic training services from the specific Athletic Trainer. If they answer “No”, the student athlete is asked to exit the survey. If yes, the survey will present questions for the student athlete to answer.
The Student-Athlete Evaluation of Faculty Athletic Trainer is comprised of 13 questions. Item 1 asks the student athlete how many times they have interacted with the Faculty Athletic Trainer. (1-5 times; 6-10 times; 11 or more times) Items 2 – 12, student athletes are asked to rate the Faculty Athletic Trainer on a scale of “5” Strongly Agree to “1” Strongly Disagree. The final item on the survey is an open-ended question inviting the student athlete to provide any additional comments regarding the Faculty Athletic Trainer.

Results reports are issued for surveys that receive a minimum of 5 student responses and 50% response rate. The results report includes the mean for the rating scale questions.

The instructor receives the quantitative results and any student comments collected.  The department evaluation committee chair receives only the quantitative results.
